APPLICATIONS AND FORMS

You can now submit your immigration-related requests to ISS through the Terra Dotta student portal in your MyUW account.

In an effort to move paperless and increase efficiency, ISS will be closing most Qualtrics forms and pdf-styled applications. On April 13th, 2020, ISS will be accepting applications through Terra Dotta. Only in special circumstances will the office accept paper forms through email. 

If you have already started to complete a paper form, these will still be accepted in the following ways until April 13th, 2020:

  • Submit your completed applications to the ISS general email
  • Or, upload your documents as a single PDF to the ISS Application Drop Box.

Application Processing

ISS processes applications on a first-come, first serve basis. Please allow a maximum 15 business days for completed applications. During busy times, such as the end of the semester, processing may take up to 15 business days. Plan in advance.

ISS will email you when we have processed your application and/or when your document is ready to be collected or mailed.

FORM PURPOSE
Change or Add Major If you have changed or added a major, use this form to update ISS.
I-20 Reprint Request Use this form if you have lost your document, have updated information, or need a travel endorsement and have run out of room on your current document.

F-1 Students - Transfers

FORM PURPOSE
F1 Transfer Out Request Use this form to request the transfer of your SEVIS record if you have been admitted for a program of study at a different institution.

The form above is on Terra Dotta. If you are having issues logging in to Terra Dotta as a OPT or STEM OPT student, please use the transfer out form within the OPT and STEM OPT drop-down menu below.

F-1 Transfer-in requests are handled by Admissions.  Check with Admissions for further guidance.

F-1 Students - OPT and STEM OPT

F-1 Students on OPT or STEM OPT

Form Purpose
OPT STEM Extension Application The STEM Extension application is only for F–1 students currently on OPT in a STEM field with a qualifying employer.
OPT and STEM Extension Reporting Form Use this form if you are on OPT or a STEM extension and have a change to report in your employment, address, or name. You must report any changes in your employment information within 10 days. Check on STEM reporting requirements here.
OPT/H1B Cap Gap I20 Request Use this form to request an I20 with cap gap remarks. You must have official USCIS documentation to make this request. 

OPT Application can be only be found on Terra Dotta. The OPT workshop is included in the application. Find more information about OPT here.

Below is the form you may use if you plan to transfer your SEVIS record to another school while on OPT or STEM OPT.

J-1 Students - Transfers

FORM PURPOSE
J-1 International Student Transfer In Request Use this form if you are a J-1 student with sponsorship from another institution and have arrangements for UW-Madison to become your sponsor. Restrictions apply.

J-1 Students - Employment

FORM PURPOSE
J-1 Academic Training Application Use this form to apply for Academic Training. It is recommended you meet inperson with an ISS Advisor to submit this form.
